Before anything else: nothing in this article is a diagnosis, and no phone or app can provide one. What consumer devices can do is surface trends in a handful of signals that clinicians already care about — and give you something concrete to bring to a doctor rather than a vague sense that something is off. That's genuinely valuable, and it's a much narrower claim than this category usually makes.
With that established, here are the signals worth watching, what each one actually indicates, and when a trend justifies a call to a doctor.
1. Resting heart rate
What it is: your heart rate at complete rest, best measured on waking before getting out of bed. Typical adult range is roughly 60–100 beats per minute, with fitter people often lower.
Why it matters: resting heart rate is associated with cardiovascular outcomes in large population studies, and — more usefully for you — it responds to training, illness, sleep debt, alcohol and stress. It's one of the few cheap signals that reflects overall physiological load.
What to watch: not the absolute number, but your own baseline and deviations from it. A sustained rise of 5–10 bpm over several weeks with no obvious explanation is worth mentioning to a doctor. A one-day spike after a bad night is nothing.
2. Heart rate variability
What it is: the variation in time between consecutive heartbeats. Higher variability generally reflects a more adaptable autonomic nervous system.
Why it matters: HRV drops in response to stress, illness, poor sleep and heavy training load — often before you consciously notice any of it.
The important caveat: HRV is enormously variable between individuals. Comparing your number to anyone else's is meaningless. It is only useful as a personal trend, and even then it's noisy enough that weekly averages beat daily readings. Treat a multi-week downward trend as a prompt to look at sleep and stress, not as a verdict.
3. Sleep duration and consistency
Why it matters: short and irregular sleep is associated with a wide range of adverse outcomes, and it's upstream of nearly everything else on this list — it moves resting heart rate, HRV, appetite regulation and glucose handling.
What to watch: consistency of timing may matter as much as duration. Going to bed and waking at wildly different times across the week is its own risk factor, independent of total hours.
Caveat: consumer sleep staging — the deep/REM/light breakdown — is far less accurate than the marketing implies. Total duration and timing are reasonably reliable; stage percentages are estimates. Don't optimise against a number your device is guessing.
4. Waist circumference
Why it matters: abdominal fat is more strongly associated with metabolic risk than overall weight or BMI. This is particularly relevant for South Asian populations, where cardiometabolic risk appears at lower BMI thresholds than in European-origin populations — which is why several Indian guidelines use lower cut-offs than international defaults.
What to watch: a tape measure at the navel, monthly. It costs nothing, needs no device, and tracks something more meaningful than the scale.
5. Blood pressure
Why it matters: hypertension is common, largely asymptomatic, and among the most consequential modifiable risk factors there is. It is also the one most likely to go undetected for years.
What to watch: this needs a real cuff — a validated upper-arm monitor, not a wrist wearable or a camera-based app, neither of which is reliable for blood pressure. Measure seated, after five minutes at rest, twice, and log both. Elevated readings across several days warrant a doctor, not an app.
6. Daily activity and sedentary time
Why it matters: low activity and prolonged uninterrupted sitting are independently associated with worse outcomes. Your phone already measures this passively and accurately enough.
What to watch: weekly average steps, and whether long sedentary blocks are being broken up. The specific number matters less than people think — see why 10,000 steps isn't the threshold it's treated as.
What phones and apps cannot do
Being explicit about this matters more than the feature list:
- They cannot diagnose anything. Not diabetes, not heart disease, not sleep apnoea. Some regulated features — certain ECG functions, for instance — can flag specific rhythm irregularities, and even those are explicitly screening prompts, not diagnoses.
- They cannot measure blood glucose or cholesterol without dedicated hardware or a blood test. Any app claiming to infer these from a camera or a wearable is making a claim the evidence does not support.
- They cannot replace blood work. Lipids, HbA1c, thyroid function, vitamin D, kidney and liver markers — the things that actually populate a clinical risk assessment — require a lab.
- They cannot account for family history, medication, or your clinical record, all of which materially change what any given reading means.
A "health risk score" in a consumer app is a composite of lifestyle signals. It is a reasonable prompt for reflection. It is not a clinical risk estimate, and any product presenting it as one is overstepping.
When to see a doctor rather than watch a trend
Some things are not trend-monitoring situations. Seek medical attention promptly for chest pain or pressure, breathlessness at rest or on mild exertion, fainting or near-fainting, sudden severe headache, one-sided weakness or speech difficulty, or persistent unexplained weight loss. An app is the wrong tool for any of these.
For the slower signals, a reasonable rule: a trend moving in the wrong direction for four or more weeks, without an obvious explanation, is worth a conversation. Bring the data — a clinician can do more with six weeks of resting heart rate than with "I've been feeling tired."
A sensible routine
- Daily, passively: steps, sleep duration and timing, resting heart rate. No effort required beyond wearing or carrying the device.
- Weekly: review averages rather than individual days.
- Monthly: waist circumference, and blood pressure if you have a cuff.
- Annually: actual blood work with a doctor. Nothing above substitutes for this, and the tracking is most useful as context for that conversation.
These signals also interact, which is why looking at any one in isolation tends to mislead — the relationships between sleep, activity, stress and nutrition carry more information than any single metric does.
The bottom line
Your phone is a reasonable early-warning system and a poor diagnostician. Used as the first — tracking a few well-chosen signals over weeks and taking the trends to a professional — it's genuinely useful. Used as the second, it produces either false reassurance or unnecessary anxiety, and both are worse than not tracking at all.
